    Find a provider that fits your needs

  • Once you've filled out a request for consultation, a Phoenix Mental Health provider will reach out to you via email to schedule an initial 15-minute phone call to explore your needs.

    A quick phone call to get you on your way

  • After the initial phone consultation, your provider will send a follow-up email with a link to register for the Patient Portal.  Once you're registered, you can find all your patient documents in one convenient location.
